Accident Coverage
Pays cash benefits for accidental injury.* Choose from a lump sum payment of up to $10,000 per accident or a hospital cash benefit of up to $6,250. Also includes an Accidental Death Benefit which pays $10,000 instead of all other benefits (or $10,000 minus any previously paid benefit) to chosen beneficiary. Issue ages 0-63.

$3,000 Accidental Death Benefit
The policy pays a one-time death benefit when death occurs from an accident. The cost is only $10 a year for the entire family. There are no rates for this product - $10 premium at the end of every year. Issue ages 18-68.
Cash Benefit Cancer Plan
Pays maximum benefit amount in cash as listed in the policy schedule up to $50,000* for written satisfactory proof of a first-time diagnosis of internal cancer or malignant melanoma. Benefit can be used for anything - income while not working, travel expenses, at-home care, experimental treatment or whatever is needed. Pays in addition to other coverage. Available for individual, single parent, or family coverage. Issue ages 0-69.
HealthGuard
The HealthGuard plan is critical illness protection which pays a lump sum benefit directly to the policyholder upon written satisfactory proof of first diagnosis. Covered illnesses include Major Organ Transplant, Heart Attack, Stroke, Total Loss of Eyesight, Total Loss of Hearing, and End Stage Renal Failure. Benefits are available in $10,000 increments, ranging from $10,000 to $50,000. Benefit amounts reduce by 50 percent at age 65. Individual and spouse coverage available.
Foundation Signature Series
The Foundation Signature Series is an innovative solution to help manage healthcare costs. The Foundation Signature Series pays out-of-pocket deductibles, copayments and coinsurance for inpatient hospital expense for individuals with current or pending group major medical or comprehensive health insurance. Select a calendar-year maximum benefit amount from $2,000 to $10,000. Optional outpatient rider available.
